We are proud to announce a new partnership with the North East NHS Foundation Trust to improve the quality of medication information provided to patients upon discharge from hospital.
This initiative will integrate MaPPs directly into the discharge process, ensuring that patients receive clear, personalized information about their medications before leaving the hospital. The partnership aims to reduce readmissions related to medication errors and improve patient outcomes.
"Discharge is a critical time for patients, especially those with complex medication regimens," said Dr. James Wilson, Chief Medical Officer at North East NHS Foundation Trust. "By providing MaPPs information at discharge, we can help patients better understand their medications and how to take them correctly."
The pilot program will begin next month in the cardiology and geriatric departments, with plans to expand to all departments by the end of the year. Initial results from similar implementations have shown a 23% reduction in medication-related readmissions.
